Submission Information

If you are interested in trying out for any of our varsity athletic programs, you must first complete the "Required Physical Forms" found below. These forms can be completed by your family physician and mailed, faxed or emailed to the Athletic Trainer. The physical exam must be completed within a 6 month window prior to the start of the athlete's sport season for the current academic year (2014-2015). These forms are required before any tryout, practice or competition process begins.

Due Dates for ALL student-athletes for the following sports...

Soccer, Cross Country, Women's Volleyball, Tennis, Men's Golf need to have the forms completed by August 9, 2016.

Basketball, Softball, Baseball, Women's Lacrosse need to have the forms completed by August 9, 2016.

Affidavit of No Insurance

In the case that a student athlete at Penn State Abington does not have his or her own personal or family medical insurance, the below affidavit of no insurance form must be on file for the student athlete.This form must be signed in front of a Notary Public and turned in with the above "Required Physical Forms".The form is required before any tryout, practice or competition process begins.

Injury/Illness Clearance Note

• If you've had surgery or a significant injury in the past year (ex. concussion, broken bone, etc.), you must have the treating physician complete the form below to serve as your clearance note along with your required medical physical forms.
• If during the season you have seen a doctor or gone to the hospital for an injury or illness you must have the treating physician complete the form below to serve as your clearance note.
